This is a "how-to" session for internal and external consultants that need to create a custom 360-feedback survey.  We will cover how 360 feedback is used in organizations, how to customize survey content to fit your client/organization, and how to finalize and launch your survey. 

Participants key takeaways:

  • How to determine what content to include in the survey (e.g., what topics to create questions for) 
  • How to write questions that result in meaningful information
  • How to choose a rating scale 
  • What to do after your draft survey is complete

About the Speakers:

John Azzara is Founder and CEO of Fast Mirror, a cloud-based, on-demand 360-degree feedback platform. Fast Mirror allows individuals and organizations to easily build and scale robust, cost-effective 360 feedback programs. Fast Mirror’s clients range in size from individual coaches to large multi-national organizations across all industries.  Prior to launching Fast Mirror John held organizational development leadership roles at Toyota and Option One Mortgage. In 2006 he founded People Talent Solutions, where he consulted to Fortune 500 companies on a variety of talent and organizational development projects including 360 feedback, leadership development, succession planning, selection/assessment, and employee surveys.  John’s areas of expertise include 360 feedback, competency modeling, performance management, change management, and HR technology. John holds a MS in industrial and organizational psychology from Alliant University. 

Eileen Azzara is an organizational development professional with internal and external consulting experience in a variety of areas including leadership development, talent assessment, organizational diagnostics, and learning technology implementation. Her areas of expertise include 360-degree feedback, competency modeling, individual and organizational assessment, talent planning and development planning. She's held internal OD program manager roles in the healthcare and utility industries and has also consulted externally for clients in a variety of sectors including financial services, manufacturing, retail, hospitality, and biotech. Currently she works in an internal organizational development consulting role for a leading dental support organization. Eileen holds a MS in industrial and organizational psychology from San Diego State University.
